  • De-identified Information. Personal Information does not include De-Identified Information. De-identified Information is data that does not identify you and for which there is not a reasonable basis to believe that it can be used to identify you.
  • Technical and Usage Data. As you navigate through and interact with our Website, we may collect information about your internet connection, the equipment you use to access our Website, and information about how you use the Website, called Technical and Usage Data. Examples of Technical and Usage Data might include your operating system; browser type; domain names; the pages within the Website you visit; access dates and times; information about what websites you traveled from to our Website; and approximate location data (geolocation).
  • Information Relating to Our Products or Services. IF YOU DO NOT CONSENT TO BEING CALLED BY A HOSPITAL OR PROVIDER REPRESENTATIVE, DO NOT ENTER YOUR PERSONAL INFORMATION INTO FORMS ON OUR WEBSITE.

Visitors to the Website who volunteer Personal Information, or who initiate a call to us by clicking on a telephone number on the Website, consent to be contacted by us, physician practice groups, or a call center. We may contact you by telephone and may, among other things, verify your self-reported Personal Information and interest in services or speaking with a physician. We may contact you by telephone even if you have opted into the National Do Not Call list, or any State Do Not Call list.

If you do not wish to be contacted again after we call you, please tell our representative on the phone that you do not want to be contacted.

We may disclose De-identified Information as permitted by law.

SOCIAL MEDIA

We may engage with you on third party social media sites like Instagram, Facebook, LinkedIn or X (formerly Twitter). If you engage with us on social media, you "opt in" to sharing your social media content with us. Be aware that the content you submit to social media sites (uploads, posts, comments, shares, "likes" and any messages or tweets) will be publicly visible. To manage the social media notifications you receive or to adjust your privacy settings on your social media accounts, please access the profile section of your social media accounts and follow the directions there.


Links to PORTALS AND Third-Party Websites

Our Website may provide you with links to portals that offer secure, private access to allow you to make payments or view your patient records. These portals may be operated by us or by our service providers and are governed by separate privacy policies.

The Website may include links to third-party websites as a resource. We do not have control over third-party websites, and those third-party websites do not operate under this Privacy Policy. We are not responsible for the content of those websites or the actions of any third parties operating or managing those websites.
